Intake Forms
Completing intake forms in advance helps ensure your visit runs smoothly and allows our team to review important information before your appointment. This supports safety, comfort, and efficient care during your session.
If you are unable to complete forms in advance, please arrive early to allow time for completion.
Arrival Guidelines
- Arrive 10–15 minutes early
- Bring valid photo ID
- Wear comfortable, loose-fitting clothing
- Bring surgical garments or discharge papers if post-op
- Hydrate before body contouring sessions
- Avoid heavy meals 2 hours prior
Post-Operative Clients
- Surgeon clearance may be required
- Bring discharge instructions if applicable
- Always follow your surgeon’s guidance
